<p><span style="font-weight: 400;">Question: “I&#8217;ve struggled with substance use in the past. Can ketamine help me stay sober?”</span></p>
<h2>Answer:</h2>
<p>Addiction is, at its core, a state of &#8220;maladaptive plasticity.&#8221; Your brain has &#8220;learned&#8221; to rely on a substance to manage stress or feel pleasure. The neural pathways for craving have become like eight-lane highways, while the pathways for self-control and healthy coping have become like overgrown dirt roads. Breaking that cycle is incredibly difficult because your brain is physically wired to seek out the substance. This is why &#8220;willpower&#8221; is often not enough.</p>
<p>Ketamine can be a powerful ally in recovery because it helps to &#8220;re-pave&#8221; the roads. During the neuroplastic window, the brain is more flexible and less &#8220;stuck&#8221; in those old patterns of craving. It provides a window of clarity where the volume of the cravings is turned down, allowing you to actually engage with your recovery tools. It&#8217;s not a magic pill that fixes addiction, but it is a tool that can help &#8220;level the playing field&#8221; so that your hard work in therapy and support groups actually sticks.</p>
<p>We are very careful and intentional when working with patients who have a history of substance use. We focus on the clinical setting and the physician-led oversight to ensure the medicine is being used for its therapeutic purpose. <p><span style="font-weight: 400;">This is Dr. Tanya Dall MD and I&#8217;ve given myself 10 minutes to answer a question about why we treat both pain and mood.</span></p>
<p><span style="font-weight: 400;">Question: “I came in for my back pain, but my mood has improved even more. Why is that?”</span></p>
<h2>Answer:</h2>
<p>This is something we see all the time, and it is not a coincidence. Physical pain and emotional pain are processed in the same areas of the brain, specifically the limbic system. When you are in chronic physical pain, those pathways are constantly on fire. This leads to a secondary &#8220;emotional&#8221; pruning where your ability to feel joy and hope is literally crowded out by the pain signals. You can&#8217;t have one without the other eventually being affected.</p>
<p>Ketamine is a &#8220;multimodal&#8221; treatment. It doesn&#8217;t know the difference between a &#8220;pain signal&#8221; and a &#8220;depression signal&#8221;; it just sees overactive NMDA receptors and &#8220;pruned&#8221; neural branches. By addressing the health of the nervous system as a whole, we are treating both conditions simultaneously. This is why we often see a &#8220;lifting of the fog&#8221; even in patients who were only seeking help for a physical injury. The brain is an integrated system, and when you heal one part, the rest often follows.</p>
<p>This is also why we include both mental and physical components in our reprogramming protocol. We want to address the whole person. If we only treated the back, the depression would eventually drag the physical health back down. If we only treated the mood, the chronic pain would eventually trigger a relapse in depression. By focusing on the health of the entire nervous system, we are building a foundation for a much more resilient and healthy life.</p>

This is the area of the brain responsible for executive function, focus, and decision making. When these connections are pruned away by cortisol and stress, you feel like you are walking through mud. <h2>Answer:</h2>
<p>The feeling of dissociation, feeling disconnected from your body or your surroundings, is one of the most common effects of ketamine. For some, it can be a bit intimidating to think about. However, in a clinical setting, we actually view this dissociation as a therapeutic tool. It is not just a side effect; it is a key part of how the medicine helps to &#8220;unstick&#8221; the brain from its current patterns.</p>
<p>Think of it this way. If you have been depressed or in pain for years, your &#8220;identity&#8221; has become merged with that suffering. Dissociation allows you to take a step back and observe your life and your thoughts without the usual emotional &#8220;weight&#8221; attached to them. It provides a perspective shift that is very difficult to achieve through talk therapy alone. It&#8217;s like stepping out of a dark room and into the light for forty minutes; you can finally see the furniture and figure out how to rearrange it.</p>
<p>We do everything we can to make this experience comfortable. We use calm music, eye masks, and a peaceful environment to sure that the &#8220;trip&#8221; is productive. Most patients describe it as a peaceful, dream-like state. Because we are right there with you, we can adjust the dose if things feel too intense. Most people find that once they &#8220;let go&#8221; and stop trying to control the experience, the dissociation becomes a profound source of insight and relief.</p>

<p><span style="font-weight: 400;">Question: “What is the difference between the IV infusions and the Spravato nasal spray, and which is better for me?”</span></p>
<h2>Answer:</h2>
<p>Both IV ketamine and Spravato are excellent tools, but they work in slightly different ways. Spravato is a nasal that is FDA approved specifically for treatment-resistant depression. Because it is FDA approved, it is often covered by insurance, which makes it a great option for many of our patients in Tustin. However, because it is a nasal spray, the absorption can vary based on your anatomy or even if you have a common cold. You also have less control over the exact dose that reaches the brain.</p>
<p>IV ketamine is what we consider the &#8220;golden standard&#8221; for precision. Because it goes directly into the vein, 100 percent of the medicine is absorbed. This allows us to be very specific with the dosage and to adjust it in real time. If you are a chronic pain patient, IV is usually the only effective route because we need much higher doses than a nasal spray can provide. For mood disorders, some patients prefer the IV because the &#8220;experience&#8221; is more consistent and the results can sometimes be more robust.</p>
<p>When you come in for a consultation, we look at your specific goals, your medical history, and your insurance situation. If your primary issue is depression and cost is a major factor, Spravato is often the best place to start. If you are dealing with complex pain or have found other treatments to be ineffective, the IV route may offer the depth of treatment you need. Our goal is to find the path that gives you the best chance of long term recovery.</p>

<p><span style="font-weight: 400;">Question: “Why do I need to do exercises after my infusion? Can&#8217;t the medicine just do the work?”</span></p>
<h2>Answer:</h2>
<p>This is a great question. Think of ketamine as a tool that opens a window. For 48 to 72 hours, your brain is in a state of neuroplasticity, meaning it is ready to grow and change. But the medicine doesn&#8217;t choose &#8220;how&#8221; the brain changes. If you get an infusion and then go home to a stressful environment and sit in the same negative thought patterns, your brain will simply &#8220;harden&#8221; back into those old shapes. You will have missed the opportunity to build something new.</p>
<p>Our reprogramming protocol is designed to take advantage of that open window. We provide you with specific, 20 minute daily exercises that give your brain a &#8220;blueprint&#8221; for how to rebuild. For pain patients, this involves movements that teach the brain that the body is safe. For mood patients, it involves cognitive tasks that strengthen positive pathways. We are being very intentional about what we feed your brain while it is in its most receptive state.</p>
<p>I often tell patients that the infusion is 50 percent of the treatment and the reprogramming is the other 50 percent. We want to move away from the idea of a &#8220;passive&#8221; patient who just receives a medicine. We want you to be an active participant in your healing. By doing the work during the neuroplasticity window, you are ensuring that the changes we make are structural and long lasting. We aren&#8217;t just looking for a temporary chemical boost; we are looking for a permanent shift in how your brain operates.</p>

<h2>Answer:</h2>
<p>Postpartum depression is a unique situation because time is of the essence. A new mother does not have six weeks to wait for a traditional antidepressant to maybe start working. <h2>Answer:</h2>
<p><span style="font-weight: 400;">Let’s get some biology straight first. I promise we will get to ketamine soon. For decades, the standard medical theory was that depression was simply a chemical imbalance. The idea was that your brain didn&#8217;t have enough serotonin or dopamine. If we just added a pill to increase those levels, you should feel better. This is the monoamine hypothesis. While it works for some, it ignores a much deeper issue: the physical structure of the brain.</span></p>
<p><span style="font-weight: 400;">When a person lives with chronic depression or stress, their brain actually undergoes a process called pruning. The branches of the neurons that allow for communication between different parts of the brain literally wither away. It is like a tree losing its leaves and small branches during a drought. If those connections are gone, you can pour as much serotonin into the system as you want, but there is no infrastructure left to receive the signal. This is why you might feel like you are just managing side effects without any real change in your mood.</span></p>

<p>Hello,</p>
<p>This is Dr Justin Yanuck MD and I’ve given myself 10 minutes to answer this patient question. This weeks question centers on how ketamine works  </p>
<h2>Answer:</h2>
<p>This is something that is easily searchable, but I will try and distill it down in a more digestible way, and focus less on the jargon of this space, and more on what most people care to know.  </p>
<p>Ketamine is a medication that has multiple uses. It is a dose dependent medicine and the way we use it in our clinic is either via the intravenous route or the nasal spray route (Spravato). The brain is full of electrical circuits called neurons. One of the primary ways ketamine works is by blocking a receptor on these electrical circuits called the NMDA receptor. For anesthesia, at high doses, this blockade of this receptor leads to the blockade of communication between certain neurons, especially in the parts of the brain tied to memory, self-awareness, and sensory integration). This allows for procedures to be done without the perception of pain or even the perception of self. Hence why ketamine is called a dissociative anesthetic. It disconnects the brains perception of the body and surroundings.</p>
<p>Now in our clinic…that is not the goal. Like many medicines, the effect of ketamine is much different at lower doses. For reference, an anesthetic dose of ketamine is 1-2mg of ketamine per kg of body weight pushed over 1-2 minutes into an IV. Whereas doses for depression, anxiety start at 0.5mg/kg of ketamine…. over 40 minutes! This lower dose of ketamine allows for a much different experience, and a much different outcome.</p>
<p>OK now to the meat of it. How does it work for mental health issues such as depression and anxiety. Let’s leave the jargon fully behind, sorry about the above part, could not avoid certain words like dissociative. So, in chronic stress states (depression, anxiety, ptsd and more) the neural pathways of the brain prune down to small, fixed channels of communication. We see this on images that track neural connections as well as multiple animal models. Think of it this way. Traumas are evolutionarily designed to be given priority in the mind. The rest is just noise. I need to know that when a hard object is coming towards me, I need to duck, otherwise I get hit. I can’t sit there and spend time processing it, trying to understand what it is, and what it might feel or taste like. Just need to duck!</p>
<p>Our minds do this all the time, and the extreme manifestation of this psychologically is these fixed negative thought spirals such as “I am worthless” or “nothing good will ever happen to me” or full dissociation, where one has zero conscious recollection of traumas that happened to them…but their bodies and unconscious mind do!</p>
<p>Ketamine rapidly expands the pruned-down neural networks. This rapid increase in neural connections allows for the ability to recollect prior memories (many of which have been suppressed) and think about things in a different way. This can manifest as recalling past traumas, or it can manifest as findings a sense of collective love and wonder at the world and the simple things in life. The key is expansion of the mind and what comes next depends largely on the work after the infusion including self-directed and with help (psychotherapy). Ultimately, this expanded consciousness opens one to leaving their current fixed thought spirals and start to more easily explore other ways of thinking about things. </p>
<p>This might either sound overly simplistic, or to others, too good to be true, but this is really the core of how this medicine works. There is some beauty in the simplicity of it. In a very real way, it is healing from within, and this is why I routinely see patients get treatment and not need treatment any further after they are done. </p>
<p>So how does ketamine work? It opens the mind. The rest is up to you. </p>
